For persons who enter the sweepstakes through this creative presentation,
these rules govern all aspects of participation and prize award.

ONLY ONE (1) WINNER ACROSS ALL WEB SITES WILL BE AWARDED FOR EACH SWEEPSTAKES PRIZE.
INTRODUCTION. This Drawing ("Sweepstakes") is sponsored by Sweepsclub.com ("Sponsor"), 999 Yamato Road, Suite 201, Boca Raton, FL 33431. The Sweepstakes is offered in conjunction with multiple companies ("Promoters") through different Web sites. One (1) winner will be selected in a random drawing from among all eligible entries received through all Web Sites. Odds of winning depend on the number of entries received in any given Web Site multiplied by the total number of Web Sites. The Sweepstakes is offered via the Internet and U.S. mail only within the United States and Canada (excluding Quebec), except where prohibited.

Timing. Each Sweepstakes has specific Start and End Dates ("Promotion Period") as indicated on this page. Various Web Sites may be added throughout the Promotion Period.

ELIGIBILITY: The Sweepstakes is open to legal residents of the United States and Canada (excluding Quebec), 18 years of age or older at the time of entry. Employees of SPONSOR and Promoters, their consultants, advertising agencies and promotional agencies, and immediate families (spouses, parents, children, siblings) and household members of such employees are not eligible. By participating, entrant agrees to abide by these Official Rules and the decisions of Sponsor, which are final and binding in all respects.

HOW TO ENTER. There are two (2) ways to enter the Sweepstakes. To enter online, visit Sweepsclub.com or a Promoter's Web site and follow the links to the entry form. Follow the instructions for entry. All online entries must be received during the Promotion Period. You may enter once a day, through as many Web Sites as you like. In the event of a dispute as to any entry, the authorized account holder of the e-mail address used to enter will be deemed to be the entrant. Potential winners may be required to show proof of being the authorized account holder. The "authorized account holder" is the natural person assigned an e-mail address by an Internet access provider, online service provider or other organization responsible for assigning e-mail addresses for the domain associated with the submitted address. Proof of sending will not be deemed to be proof of receipt by Sponsor.
To enter by mail, hand print your name, address, day and evening phone numbers, e-mail address (if any), date of birth, the name of one of the Sweepstakes Promoters and the prize offered by that Promoter on a 3" x 5" piece of paper. Mail it in a hand-addressed envelope to: Syndicated Sweepstakes Drawing, Sweepsclub.com, 999 Yamato Road, Suite 201, Boca Raton, FL 33431. All mail-in entries must be received by the End Date for that specific sweepstakes. You may enter as many times as you like, but there is a limit of one (1) entry per separately mailed envelope. All entries become the exclusive property of the Sponsor and the applicable Promoter, and no entry will be acknowledged or returned. Proof of sending will not be deemed to be proof of receipt by Sponsor.

CONDITIONS OF ENTRY: By entering this Sweepstakes, you agree to be bound by these Official Rules and by the decisions of the judges, which shall be final and binding in all respects. You also consent to the use of your name and/or likeness by SPONSOR and its Promoters for promotional purposes, including online announcements, without additional compensation, except where prohibited by law. Neither SPONSOR, the Promoters, its consultants, agencies or employees nor any telephone network or service provider is/are responsible for incorrect, inaccurate or incomplete transcription of entry information; for lost, stolen, late, or misdirected entries; for destruction or alteration of entries; for unauthorized access to entries; or for any human error, technical malfunction, delay in operation or transmission, lost data, omission, interruption, deletion, defect, communications line failure, computer equipment, software, inability to access any Web site or online service, or any other error or malfunction.

SELECTION OF POTENTIAL WINNER. Approximately 14 days from the End Date of this Sweepstakes, Sponsor will randomly select one (1) winner from among all eligible entries received from all Web Sites. Entrant selected must comply with all terms and conditions of these Official Rules, and winning is contingent upon fulfilling all requirements. Odds of winning depend upon the number of eligible entries received from all Web Sites. Purchasing a product from any of SPONSOR's Sponsors or Promoters will not improve your chances of winning this sweepstakes. Potential winner will be notified by e-mail message, at which time his/her address will be confirmed. Potential winner has fourteen (14) days to respond to e-mail, after which time, potential winner forfeits his/her prize and a new potential winner will be selected from among all eligible entries. SPONSOR and its Promoters assume no responsibility for an incorrect e-mail address, bounced-back e-mail messages, mechanical or technical failure of any kind, or lost, late or misdirected e-mail. Return of any prize notification as undeliverable by e-mail will result in disqualification and the selection of an alternate potential winner. Entrants are encouraged to print out a copy of these Official Rules for future reference. All taxes (if any) are the sole responsibility of the winner. No prize substitution, cash substitution, or assignment or transfer of prizes is permitted, except by SPONSOR because of prize unavailability, in which case a prize of equal or greater value will be awarded.

PRIZE: A separate sweepstakes is conducted for each of the prizes identified immediately above these Official Rules. Entrant elects which sweepstakes he/she is entering by choosing to enter at a Web Site for the particular prize he/she would like to win. The identities and approximate values of the prizes set forth immediately below these Official Rules are incorporated herein by reference. One (1) Grand prize will be awarded for each sweepstakes. The prize for which an entrant is eligible for depends on the prize listed that day (if entry was online) or that he/she listed on his/her mail-in entry form. Winner will be selected in a random drawing from among all eligible entries received. Sweepsclub.com sometimes purchases products, gift certificates or services to be awarded as prizes in the promotions featured at this site or on Promoters sites. There is no affiliation between Sweepsclub.com and such product manufacturers, retailers, or service providers; nor have those companies endorsed Sweepsclub.com or the promotions in question. Trademarks of companies other than Sweepsclub.com, when used in connection with promotions featured at this site, are the intellectual property of the trademark owners and are used at this site for identification purposes only.

GENERAL CONDITIONS: This sweepstakes is subject to all applicable federal, state, provincial and local laws. Void where prohibited. Entrants hereby agree that SPONSOR, the Promoters, their consultants, agencies and employees shall not be liable for any injury, loss or damage of any kind (including but not limited to damage to computer equipment) resulting from participation in this sweepstakes, downloading, or the acceptance or use of any prize awarded. SPONSOR reserves the right to verify eligibility qualifications of any potential winner. Potential winner will be required to execute an affidavit of eligibility and a liability/ publicity release within fourteen (14) days of attempted notification. Failure to comply within this time period may result in disqualification and awarding of the prize to an alternate entrant. Canadian residents must correctly answer a time-limited skill-testing question to win a prize. SPONSOR is not responsible for typographical or other errors in the offer or administration of this sweepstakes, including but not limited to errors in advertising, Promoter's sites, the Official Rules, the selection and announcement of the winner of any prize or the distribution of any prize. SPONSOR reserves the right to modify these rules for clarification purposes without materially affecting the terms and conditions of the sweepstakes. If for any reason the computerized and Internet portion of this sweepstakes is incapable of running as planned because of infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure, strike, act of God, or any other cause beyond the control of SPONSOR, SPONSOR reserves the right, at its sole discretion and without obligation or prior notice, to cancel, terminate, modify or suspend this sweepstakes. In such event, SPONSOR may conduct a drawing to award the prize using all eligible entries received as of the time of cancellation, termination, modification or suspension. SPONSOR reserves the right to disqualify any individual who tampers with the entry process or any other aspect of the Sweepstakes.

PRIVACY: Personal information is collected from entrants when they participate in this sweepstakes. Please see the Sweepsclub.com Privacy Policy at www.Sweepsclub.com. WINNER'S NAME: Winner's name will be posted on the Winners page at www.Sweepsclub.com and linked from all Promoters Web Sites for a minimum of 14 days from the sweepstakes End Date, after which it may be removed. 1. INTRODUCTION: "The Choice is Yours II" Sweepstakes is sponsored by Equifax Marketing Services, 999 Yamato Road, Suite 201, Boca Raton, FL 33431. Internet SurveyPanel.com and SweepsClub.com are web sites owned and operated by Equifax Marketing Services. The sweepstakes is offered to legal residents of the United States and Canada (excluding Quebec), as confirmed by a valid government-issued photo identification.

2. SWEEPSTAKES PERIOD: Entries for "The Choice is Yours II" Sweepstakes may be submitted beginning on June 16, 2004 at 12:01 a.m. Eastern Daylight Savings Time, except for residents of Florida, Rhode Island and New York, who may enter only after July 16, 2004 at 12:01 a.m. Eastern Daylight Savings Time. Drawing to determine the winner of "The Choice is Yours II" Sweepstakes will take place on or around December 15, 2005. To be eligible for "The Choice is Yours II" drawing, entries must be received by Equifax Marketing Services by November 30, 2005 at 11:59 p.m. Eastern Standard Time. By participating, you agree to these Official Rules. Other restrictions apply.

3. PRIZE: Equifax Marketing Services will award one (1) automobile to one (1) winner of "The Choice is Yours II" Sweepstakes whose name is randomly drawn from all entries received in accordance with paragraph 6 of these Official Rules and who is otherwise eligible. The automobile will be selected by the winner from a list of three (3) choices: a Nissan 350Z Coupe (6 speed manual transmission) (approximate retail value is $26,930), a Ford Escape Hybrid (approximate retail value is $29,500), or a BMW 325i Sedan (5 speed steptronic transmission) (approximate retail value is $28,100).

4. PRIZE RESTRICTIONS: If winner selects the vehicle of lesser value, he or she will not be entitled to payment or compensation for the difference in value between the three vehicles. Winner is solely responsible for pick-up or delivery of the vehicle from automobile dealership, whose location will be determined by Equifax Marketing Services. Vehicle manufacturer, make, model and trim level are subject to change. Color and options of vehicle selected by winner are subject to change. All taxes, title, tag, registration, customs, duty, import, destination, and brokerage fees (if any) are the sole responsibility of the winner. No prize substitution, cash substitution, assignment or transfer of the prize is permitted, except that Equifax Marketing Services may substitute a prize of equal or greater value in the event it deems necessary. There is no warranty of any type on the vehicle awarded except as may be offered by the manufacturer or as otherwise required by law. Potential winner must have both a valid license to operate the motor vehicle and proof of insurance at the time the prize is awarded.

5. ELIGIBILITY: "The Choice is Yours II" Sweepstakes is open only to legal residents of the United States and Canada (excluding Quebec) who are 18 years of age or older at the time of entry as confirmed by a valid government-issued photo identification. Employees of Equifax, Inc., or any of their subsidiaries, and the immediate families (spouses, parents, children, siblings) and household members of such employees are not eligible.

6. HOW TO ENTER: NO PURCHASE NECESSARY. Purchasing a product from Equifax Marketing Services or any of its affiliates or marketing partners will not improve your chances of winning. There are three ways to enter:
   
   1. If you are a SWEEPSCLUB.COM member, you may enter online by visiting www.SweepsClub.com and using points in your Reward Points Program account to enter the sweepstakes. To do so, access the Syndicated Points for Sweeps landing page. Each sweepstakes entry will reduce the balance in your Reward Points Program account by 1,000 points. For example, if you submit 2 entries over two days, you will be entered twice and 2,000 points will be deducted from your account. Membership in SWEEPSCLUB.COM is free.
   2. You may enter online by accessing any of the following web sites and completing all required fields of their respective online entry forms:

      3. Additional company owned websites, or partner properties, that may be added in the future.
   3. You may enter by printing your first and last name, complete address, e-mail address, and day and evening phone numbers on a 3" x 5" card and mailing the card in an envelope by U.S. or Canadian mail to: SWEEPSCLUB.COM c/o Equifax Marketing Services, Department "Entries", P.O. Box 7045 Box A Delray Beach, FL 33482. Please specify on the 3" x 5" card that you are entering "The Choice is Yours II" Sweepstakes. Limit 1 entry card per [outer] mail envelope.
   
   If you are entering the sweepstakes via any online method, please remember to set your browser to accept cookies. You may submit no more than 1 entry (by any combination of entry methods above) per day. You may not exceed this limit by entering under different e-mail addresses. Entries are not valid unless the entry form or card is completed properly and a valid e-mail address is provided. Entries are the property of Equifax Marketing Services and will not be returned.

7. SELECTION OF WINNER: One (1) winner will be selected in a random drawing from all entries received on December 15, 2005, or within a reasonable time thereafter. Odds of winning depend upon the number of entries received. Winner will be notified by certified mail, return receipt requested, addressed to the home address provided on winner's entry form. Winner also will be notified via electronic mail, at which time his/her e-mail address will be confirmed. Before receiving any prize and as a condition of receiving such prize, the winner must provide a valid government-issued photo identification featuring his or her name, date of birth, and address as it appeared on the entry form supplied, provide his or her social security number, offer proof of proper age and residency in any state in the U.S. or Canada (excluding Quebec), and sign (without making any changes) an affidavit of eligibility and release supplied by Equifax Marketing Services which, among other disclaimers, releases Equifax, Inc., and a range of subsidiaries, directors, agents, agencies, affiliates, advertisers, promoters, officers, directors, employees and related persons from any and all liability related to this Sweepstakes and the receipt and use of the prize. In addition to the above requirements, Canadian residents also must answer correctly a time-limited skill-testing question to win a prize. Winner also may be required to sign other legal documents (including tax forms). If the winner fails to sign and return the required documents within fourteen (14) days of the date on the notification letter, does not accept the prize, or fails to meet any eligibility requirement herein, an alternate winner will be chosen by random drawing to be held as soon as reasonably practicable thereafter. Alternate winner is subject to all eligibility requirements and restrictions of these Official Rules. Equifax Marketing Services will furnish to the Internal Revenue Service or the Canadian Customs and Revenue Agency any required tax reporting forms relating to the award of the prize including Form 1099-MISC.

8. MISCELLANEOUS: This sweepstakes is subject to all applicable federal, state, provincial and local laws. Void where prohibited. Equifax Marketing Services shall be entitled to interpret these rules as needed - including but not limited to rules regarding entries, selection of winner, deadlines, restrictions on the prize, and eligibility - and all of its decisions are final. By entering, you agree to these conditions. You agree that Equifax Inc., their subsidiaries and affiliates, and their consultants, agencies and employees, and any telephone network or service provider shall not be responsible for incorrect, inaccurate or incomplete transcription of entry information; for lost, stolen, late, or misdirected entries; for incorrect e-mail addresses or bounced-back e-mail messages; for destruction or alteration of entries; for unauthorized access to entries; or for any human error, technical malfunction, delay in operation or transmission, lost data, omission, interruption, deletion, defect, communications line failure, computer equipment, software, inability to access any web site or online service, or any other error or malfunction. Proof of sending an entry will not be deemed to be proof of receipt by Equifax Marketing Services.

VIOLATIONS OF RULES: Equifax Marketing Services reserves the right, at its sole discretion, to disqualify any individual it reasonably suspects or believes to be tampering with the entry process or the award mechanism, whether on-line or by any other means, or to be acting in violation of these rules. A violation of the rules includes any attempt to enter more than 1 time per day. Any person attempting to defraud or tamper with this Sweepstakes in any way will be ineligible for prizes and may be prosecuted to the full extent of the law.

11. LIMITATION ON LIABILITY: You agree that Equifax Inc., and their subsidiaries, affiliates, consultants, agents and employees and any telephone network or service provider will not be liable for any injury, loss or damage of any kind (including but not limited to damage to computer equipment) resulting from participation in this sweepstakes, downloading, or the acceptance or use of the prize. You further agree that Equifax Inc., and their subsidiaries and affiliates are not responsible for typographical or other errors in the offer or administration of this sweepstakes, including but not limited to errors in advertising, the Official Rules, the selection and announcement of the winner or the distribution of the prize. Equifax Marketing Services reserves the right to modify these rules for clarification purposes without materially affecting the terms and conditions of the sweepstakes. 12. IMPAIRMENT OF INTEGRITY OF SWEEPSTAKES: If for any reason this sweepstakes or any portion thereof is incapable of running as planned because of infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure, strike, act of God, or any other cause beyond the control of Equifax Marketing Services, Equifax Marketing Services reserves the right, at its sole discretion and without obligation or prior notice, to cancel, terminate, modify or suspend the affected part of the sweepstakes. In such event, Equifax Marketing Services will conduct a drawing to award the prize using all entries received as of the time of cancellation, termination, modification or suspension.

13. CONSENT TO USE OF INFORMATION. You specifically acknowledge and agree that Equifax, Inc., and their subsidiaries, affiliates and selected third parties may utilize information obtained from you in order to contact you and/or to offer their products or services or for other promotional purposes, including online announcements, and as otherwise permitted by their privacy polices. For further information, please refer to the Privacy Statement for the website through which you submitted your entry to "The Choice is Yours II" Sweepstakes. If you would like to have your name removed from future mailings by Equifax Marketing Services, you may notify Equifax Marketing Services at the following address: 999 Yamato Road, Suite 201, Boca Raton, FL 33431. By accepting a prize, winner consents to the use of his or her name, photograph (including the use and appearance of winner's photograph on the web pages of Equifax, Inc., and its affiliates and subsidiaries), likeness, biographical information, voice, and e-mail address in this and future promotions and publicity without further compensation, except where prohibited by law.

14. WINNER'S NAME: Winner's name will be posted on the Winners' page at http://www.internetsurveypanel.com/ for a minimum of 14 days, after which it may be removed. The Winner's name also may be posted on the other web sites listed above in paragraph 6(b).
